A Story About a Cyst, a Blood Test, and a Very Long Night

Last month, a woman walked into my clinic with her husband, her sister, and a folder of reports held so tightly it had creased at the corners.
She sat down before I even said a word and placed the folder on my table.
“Doctor, please tell us honestly. Is it cancer?”
Her ultrasound had shown a “complex adnexal cyst.” Somewhere along the way, someone had also sent a CA-125 test, and the number had come back a little above the normal range, printed in red. That was three days ago. In those three days, she told me later, she had barely slept. Her husband had gone online at 2 a.m. and read enough to convince himself the worst was already decided. Her sister had come along “just in case.”
I asked her gently — how old are you, when was your last period, do you have any pain, any bloating, any change in appetite. She was 34. Her periods were regular. She’d actually come for the scan because of mild period pain, nothing more.
I looked at the report properly. The cyst had thin walls, a bit of internal texture, but no solid nodules, no concerning blood flow, no fluid elsewhere in the abdomen. In someone her age, with that history, this pattern is far more often something like a functional cyst or early endometriosis than anything sinister.
Then we came to the number she’d been carrying in her head for three days.
“Doctor, but CA-125 is a cancer test, no? Why is it high if it’s not cancer?”
This is the question I hear almost every single week, in some form. And it’s such a reasonable question, because that’s exactly how the test gets talked about — as the cancer marker. But it isn’t that simple.
CA-125 rises in all sorts of ordinary situations. Periods can raise it. Fibroids can raise it. Pelvic infections can raise it. And very commonly, endometriosis raises it — sometimes quite a lot. In a 34-year-old with a cyst that looks the way hers did, a mildly raised CA-125 was not surprising to me at all. It didn’t change the picture; it simply fit into it.
I explained that we don’t read any one report alone — not the scan by itself, not the blood test by itself. We look at her age, whether she’s had menopause or not, exactly what the cyst looks like on ultrasound, her symptoms, and the blood work, all together. That combination is what tells us how worried to actually be — not one red number on a page.
By the end of that conversation, her shoulders had dropped from around her ears to somewhere near normal. Her plan was simple: no surgery, no hospital admission — just a repeat scan in six weeks to make sure the cyst was settling, as most do. Her husband, who’d walked in braced for the worst, asked if I could tell his mother that everything is fine. Her sister finally let go of the strap of her bag.
I think about that family often, because they are not unusual. I see some version of this scene almost every week — a person and their whole worried family, arriving with a report and a sentence already forming in their heads before I’ve even opened the file.
Here is what I wish more people knew before that sentence forms:
An ovarian cyst is not, by itself, a diagnosis of cancer. Most cysts — even ones that look a little complex on a scan — turn out to be completely harmless. And CA-125 being high does not automatically mean cancer either; in younger women especially, it’s far more often explained by something benign.
None of this means an abnormal report should be ignored — it shouldn’t. It simply means it deserves to be read properly, in context, by someone who can put the whole picture together — not carried around for three sleepless nights and a hundred google searches first.

Ovarian Cysts and Adnexal Masses: How Doctors Decide If It’s Cancer or Not
If you’ve been told you have an “ovarian cyst” or an “adnexal mass” after an ultrasound, the word that probably jumped out at you is mass (or cyst) — and the question that followed was almost certainly, is this cancer?
It’s a fair question, and a common one. Ovarian cysts and adnexal masses (growths in or near the ovary, fallopian tube, or nearby tissue) are picked up on scans very often — during routine checkups, pregnancy scans, or while investigating pelvic pain. The reassuring truth is that the vast majority turn out to be non-cancerous. But because ovarian cancer can be hard to catch early, every such finding deserves a careful, step-by-step evaluation — rather than a wait-and-watch shrug on one hand, or immediate panic on the other.
Here’s how a gynaecologic oncologist actually thinks through that question.
Step 1: Who is the patient?
Age and menopausal status change the odds enormously. A simple, thin-walled cyst in a 25-year-old is very likely a functional cyst related to ovulation and will often resolve on its own in a few cycles. The same-looking cyst in a 60-year-old postmenopausal woman carries a different level of concern, simply because functional cysts shouldn’t really be forming after menopause. Family history (especially breast or ovarian cancer in close relatives), personal history of endometriosis, and symptoms like unexplained weight loss, bloating, or early satiety also shift the risk assessment.
Step 2: What does the ultrasound actually show?
This is where a lot of the real diagnostic work happens. Radiologists and gynaecologists look at specific features of the mass rather than just its size:
- Simple vs complex: A simple cyst is a thin-walled, fluid-filled sac with nothing else inside it — reassuring. A complex mass has solid areas, thick septations (internal walls), or irregular projections — these raise concern.
- Solid components and blood flow: Solid nodules within a cyst, especially ones with blood flow on Doppler imaging, are a red flag.
- Bilaterality: Masses in both ovaries are somewhat more concerning than a one-sided finding.
- Ascites: Fluid in the abdominal cavity alongside an adnexal mass is a significant warning sign.
Structured scoring systems like IOTA (International Ovarian Tumor Analysis) ADNEX or the older simple rules help standardize this — they take these ultrasound features and generate a risk category, so the assessment isn’t purely subjective.
Step 3: Blood tests — and their limits
CA-125 is the test most patients have heard of, but it’s widely misunderstood. It’s genuinely useful in postmenopausal women with a complex mass, where an elevated level adds real weight to a cancer suspicion. In premenopausal women, though, CA-125 can be elevated by all sorts of benign, common conditions — endometriosis, fibroids, pelvic infection, even menstruation — so a raised value alone should never be treated as a cancer diagnosis in this age group.
Newer markers like HE4, and combined indices like the ROMA score (Risk of Ovarian Malignancy Algorithm) or RMI (Risk of Malignancy Index, which combines menopausal status, ultrasound score, and CA-125), help sharpen this further, particularly in borderline cases.
Step 4: Putting it together — risk stratification
None of these tools work in isolation. A gynaecologic oncologist combines age, symptoms, ultrasound architecture, and blood markers to place a mass into a rough risk category: low, intermediate, or high risk of malignancy. This determines what happens next:
- Low risk (e.g., a simple cyst in a young woman): often just a repeat scan in 6–12 weeks, since many resolve spontaneously.
- Intermediate risk: closer monitoring, sometimes an MRI for more detail.
- High risk: Planned surgery by gynaecological oncologist, ideally before any operation is done — because how a suspicious mass is handled surgically (avoiding rupture, proper staging if cancer is confirmed) matters enormously for outcomes.
Why the referral pathway matters
This is a point I’d really like patients to take away: if an adnexal mass is flagged as intermediate or high risk, the single most important decision is who operates, not just whether to operate. Studies consistently show that women with ovarian cancer who are operated on by a gynaecologic oncologist — rather than a general gynaecologist or surgeon — have better staging accuracy and better survival outcomes. If your doctor suspects a higher-risk mass, it’s entirely reasonable to ask for a referral to a specialist before surgery, not after.
The bottom line
Most adnexal masses are benign, and most don’t need surgery at all — just sensible follow-up. But “most” isn’t “all,” which is exactly why a structured evaluation exists. If you’ve been told you have an adnexal mass, the right next questions to ask your doctor are: What does the ultrasound show specifically? Do I need a CA-125 or other tumor markers? And based on all of this, what’s my risk category? Those answers, not the word “mass” by itself, are what actually tell you where you stand.
